Adoration Home Health is currently seeking a PRN MSW - Social Worker to support our home health patients in Tellico Plains, Tennessee. The home health Social Worker will make intermittent home-visits to our skilled nursing patients in a particular area. Enjoy the flexible scheduling of home heatlh!

Responsibilities

• Guides and counsels individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
• Assists clients in obtaining government funds, education, or treatment
• Secures information, such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities Asks clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem
• Helps patient and family through individual or group conferences regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioning

Qualifications

• Master's Degree in Social Work
• Five plus years clinical social work experience
• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
• Ability to make psycho-social assessments and develop and implement viable care plans
