Your Role:

The Hospice Social Worker (BSW) will be a vital member of our interdisciplinary team, providing essential psychosocial support to patients and their families. You will assess individual needs, set goals, and implement interventions that are in alignment with our established plan of care. Using your professional expertise and personal judgment, you will monitor the psychosocial assessment process effectively.

Required Qualifications:

• Bachelor's Degree in Social Work from a CSWE accredited school.
• State licensure as required.
• A minimum of one year of experience as a Social Worker in a healthcare or hospice setting; three to five years of experience is preferred.
• In-depth understanding of terminally ill patients and their families, alongside knowledge of hospice care and the psychosocial dynamics of illness, loss, and death.
• Ability to support families during times of crisis and handle the emotional impact of loss with compassion and tact.
