Your Role:

  • Conduct timely psychosocial assessments for patients and their families/caregivers within the hospice program, ensuring all identified needs inform the patient’s care plan.
  • Provide exceptional service to patients, families, referral sources, and coworkers, assisting team members to maintain a high standard of care.
  • Offer counseling to patients and families/caregivers and staff regarding stress and coping challenges.
  • Ensure accurate and timely documentation of patient services is maintained to reflect the care plan.
  • Assist with applications for Supplemental Security Income, Hospice Benefits, Medicaid, and Medicaid Waivers, and connect families with community resources.
  • Act as a liaison between patients, families/caregivers, and community agencies to facilitate support and services.

Qualifications:

  • A degree in Social Work is required, with a Master's degree preferred. A bachelor’s degree in Psychology, Sociology, or a related field is also acceptable.
  • Current State Licensure in Social Work (LSW, LISW, or LPSW-S) is required.
  • A minimum of one year of experience in a healthcare or hospice setting is necessary.
  • A valid driver's license and proof of insurance are essential.
