Case Manager Social Work - Inpatient Rehab - PRN at Norman Regional Health System summary:
The Case Manager in Social Work provides clinical care coordination, transition management, and psychosocial support within an inpatient rehab setting. This role involves advocacy, resource and financial management, utilization review, and participation in quality and performance outcome initiatives. A Bachelor’s or Master’s degree in Social Work with relevant licensure and Basic Life Support certification is required for effective patient care management.

- Clinical Care Management/Coordination/Transition Management.
- Resource/Financial Management.
- Psychosocial Management.
- Utilization Management.
- Quality/Performance & Outcomes Management.
Education
- Requires a MSW or Bachelor’s degree (BSW) in Social Work; with 2 years of experience will be accepted in lieu of the MSW.
- Either degree must be from a university accredited by Council of Social Work Education.
Experience
- Prefer 1 year of experience if MSW or 2 years of experience required if BSW.
- Medical Social Work experience preferred.
Licensure/Certification
- Licensure by Oklahoma State Board of Licensed Social Workers preferred.
- Basic Life Support (BLS) training or re-training is required during orientation prior to providing patient care.
- Current Basic Life Support (BLS) is required to be maintained thru re-training for duration of employment
Compensation
- $23.79-$38.86/hr depending on previous work experience.
