About Job

Senior Options, Inc. is seeking a compassionate and organized Care Manager to join our dedicated team. The Care Manager is responsible for assessing, planning, coordinating, and overseeing the care of older adults to ensure their health, safety, independence, and quality of life. This role requires strong interpersonal and communication skills, attention to detail, and the ability to advocate effectively for clients and their families.

Responsibilities:

  • Conduct comprehensive assessments to identify client needs, preferences, and resources.

  • Develop individualized care plans that reflect client goals and ensure quality outcomes.

  • Coordinate services across healthcare providers, community resources, and support networks.

  • Regularly monitor and evaluate care plans, making adjustments as necessary.

  • Provide education, support, and advocacy to clients and families navigating elder care challenges.

  • Maintain accurate documentation and records in compliance with agency standards and regulatory requirements.

  • Respond promptly and effectively to client needs, concerns, and emergencies.

  • Collaborate with interdisciplinary teams to ensure holistic, person-centered care.

Qualifications:

  • Bachelor’s degree in Social Work, Nursing, Gerontology, or a related field; Master’s degree preferred.

  • Minimum of 3 years of experience in care management, elder care, social services, or healthcare settings.

  • Strong understanding of aging-related issues, healthcare systems, and community resources.

  • Exceptional organizational, communication, and problem-solving skills.

  • Ability to work independently and manage multiple priorities effectively.

  • Empathy, patience, and commitment to advocating for older adults and their families.

  • Valid driver's license and reliable transportation for home visits.
