We are seeking a dynamic and results-driven Home Health Marketer to join our growing team. This individual will be responsible for promoting our home health services by developing strong community relationships, driving patient referrals, and enhancing our agency’s presence across healthcare and local networks. The ideal candidate will have a passion for patient care, excellent communication skills, and a proven track record in healthcare marketing or community outreach.
Key Responsibilities:
- Patient Referral Development:
- Identify and develop relationships with referral sources such as hospitals, physicians, case managers, discharge planners, skilled nursing facilities, assisted living facilities, and community centers.
- Educate referral sources on the agency's services, value, and patient care outcomes.
- Consistently generate qualified patient referrals for home health services.
- Community Outreach & Public Relations:
- Organize and attend health fairs, senior expos, wellness events, and community functions.
- Build and maintain relationships with community organizations, faith-based groups, senior living communities, and other relevant entities.
- Serve as a liaison between the agency and the community to promote awareness of services.
- Brand Development & Promotion:
- Create marketing materials, digital content, and community presentations that effectively communicate the mission and offerings of the agency.
- Maintain an active presence on social media platforms and collaborate on campaigns that increase visibility.
- Strategic Planning:
- Develop and execute marketing plans and strategies to meet agency goals for growth and patient census.
- Track and analyze referral trends, community feedback, and marketing effectiveness.
- Work with leadership to evaluate and improve outreach efforts continuously.
- Relationship Management:
- Provide exceptional customer service to referral sources and families.
- Maintain ongoing communication and follow-up with contacts to ensure satisfaction and future engagement.
- Monitor patient experience and ensure feedback is communicated internally.
- Compliance and Ethics:
- Adhere to all state, federal, and company compliance standards regarding marketing practices.
- Ensure all marketing activities are in line with HIPAA and regulatory guidelines.
Qualifications:
- Bachelor’s degree in Marketing, Communications, Healthcare Administration, or a related field (preferred).
- Minimum 2 years of experience in healthcare marketing, preferably in home health, hospice, or related settings.
- Proven ability to build and maintain professional relationships.
- Strong knowledge of home health regulations, services, and patient eligibility.
- Excellent verbal and written communication skills.
- Self-motivated, organized, and results-oriented.
- Valid driver’s license and reliable transportation required.
Performance Expectations:
- Meet or exceed monthly patient referral goals.
- Increase community awareness and engagement with the agency.
- Develop and maintain at least 10-15 active referral relationships.
- Maintain accurate records of outreach activities and outcomes.

Pay: $62,000.00 - $70,000.00 per year
