Job Description
About Bridge to Home
Bridge to Home (BTH) is a new innovative program developed by NYC Health + Hospitals that will provide treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) who are ready for discharge from the hospital but do not have immediate access to stable housing.
In this voluntary program, BTH will offer its guests onsite ambulatory behavioral health treatment and comprehensive support in a home-like environment. The program aims to keep individuals discharged from the hospital on a path towards sustained success, with the goal of reducing emergency room visits and inpatient hospitalizations, decreasing street homelessness and reliance on shelters, and ultimately moving into permanent housing. Program guests will stay at the home-like environment until they can be transitioned to permanent supportive housing and treatment for a period of up to 12 months.
Summary of Duties and Responsibilities
- Assumes 24/7 nursing responsibility
- Supervises nursing staff at Bridge to Home
- Ensures staffing plans meet daily operational needs and coordinates work schedules
- Provides performance appraisal, and approves timesheet for nursing staff (direct reports)
- Interprets and implements policies and procedures
- Attends meetings, keeps records and provides in-service to nursing staff onsite
- Offers limited medical services
- Assess patients based on interviews, chart reviews, and/or referrals
- Meet with patients onsite and throughout the building
- Educate and teach patients based on diagnosis and need
- Administers medication as needed
- Initiates appropriate nursing interventions based upon assessment and/or clinical guidance, which may include:
- Instructing patients and/or family members regarding treatment, medication, diagnostic procedures, follow-up appointments
- Observing and identifying underlying/contributing factors, especially socio-economic, cultural, physical and language barriers
- Assist in obtaining and maintaining an accurate list of medications and ensuring patients’ understanding of those prescriptions
- Follows protocols for securing emergency equipment and medication
- Coordinates response to medical and psychiatric emergencies.
- Schedules appointments for nurse follow-up visits (e.g., wound care, medication education) and assists with accessing specialty services as appropriate
- Triage patients as needed
- Perform vital sign checks
- Documents all observations, evaluations and interventions in the indicated forms or electronic medal records system.
- Escalates to emergency services, and follows chain of command reporting
- Investigates and follows up with complaints provides report of findings
- Schedules and manages both nurses and ancillary nursing staff
- Other duties as determined by the BTH Psychiatrist or other equivalent manager
Minimum Qualifications
1. Licensed and currently registered to practice as a professional nurse in New York State, plus (2) or (3), below.
2. Masters Degree in Nursing Administration, Education, Clinical Specialty or equivalent field and five years of satisfactory nursing experience, of which three years shall have been in a supervisory or teaching capacity; or a Baccalaureate Degree in Nursing and six years of satisfactory nursing experience, four years of which shall have been in a supervisory or teaching capacity.
3. A satisfactory equivalent of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
Department Preferences
- Lived experience or prior work in healthcare settings caring for people with experiences of homelessness, mental illness (including substance use disorders) or justice involvement strongly preferred. If there is no prior substantial experience, candidate should possess a strong desire to learn and care for this population
- Wound care experience
- Experience with naloxone (Narcan) administration and patient education
- Excellent written and verbal communication skills
- Exhibits sensitivity to the needs of a culturally and linguistically diverse patient population
- Bilingual in English and Spanish (or another language)
