Job Duties

Carries the Viral Illnesses program cell phone during business hours and responds to patient calls and messages in a timely manner.


Coordinates with patient navigators from other service lines to ensure prompt linkage to initial appointments for evaluation and initiation of treatment when appropriate. Provides information regarding directions and what to expect.


Maintains a secure spreadsheet related to positive viral testing results, linkage activites, and patient outcomes.


Ensures patients are adherent to medical appointments and medication refills; assists in coordinating refills and/or addressing medical needs as needed.


Screens patients testing negative for viral illnesses for appropriateness for linkage to comprehensive prevention services.


Provides referrals to community resources as needed, and follows up to ensure referral completion.


Prepares reports of monthly testing estimates, testing data, and linkage to care data for the FOCUS grant.


Completes medication prior authorizations for insured patients and drug manufacturer patient assistance program applications for uninsured/underinsured patients.


Maintain effective communication with patients, families, healthcare providers, and community partners.


Conduct outreach activities to identify individuals who may be at risk for or living with viral illnesses, including participation in community events, health fairs, and targeted outreach initiatives.


Other Duties

Performs other duties as assigned to support the overall objectives of the department and organization.


Position Qualifications

Experience working in a medical or social services environment preferred.

Strong attention to detail and communication and/or interpersonal skills

Ability to develop and maintain data collection tools and processes.

Proficient with basic computer programs, particularly MS Excel.

Analytical problem-solving skills.

Ability to manage time, prioritize, and meet deadlines.

Ability to work independently and with a multidisciplinary team.

Demonstrates and maintains professionalism that aligns with Code of Ethics.

Proficiency in Spanish preferred.


Education

High school graduate or equivalent required.

Some college or college degree preferred.


Salary commensurate with experience within posted range.

$45,000.00- $65,000/ Year
