• Make an Impact! Our Social Workers play an integral role on the Care Management team for our patients! In this position, you would provide a comprehensive assessment of and intervention with patients in the hospital in order to: (1) address the full range of psychosocial needs presented by patients including those who present with medical, psychiatric and/or chemical-dependency issues; and (2) ensure appropriate disposition of inpatients to the proper setting in a rehabilitation hospital, skilled nursing facility or a psychiatric facility, (3) assumes responsibility for facilitating routine returns to skilled nursing facilities when the patient has a new level of care or requires additional insurance pre-certification, (4) assist with discharge to home with the Main Line Health Home Care Provider pending the patient’s/family’s choice.

Position: Social Worker (MSW)
Shift: Part-time, 24 hours per week. Tuesday and Fridays are a must for the Clinic days.\

Experience:
Minimum of one year's experience in comparable healthcare setting. Oncology experience a plus.

Education:
Master of Social Work

Licensures/Certifications:
L.S.W. preferred
