Job Summary
The company is a rapidly growing Long Island based licensed home care agency, The Home Care Coordinator plays a pivotal role in ensuring seamless communication and coordination between patients, healthcare contracts, and administrative staff. This position is essential for managing patient care processes, facilitating coverage of services and maintaining accurate records. The ideal candidate will possess strong organizational skills, a solid understanding of HHA Exchange, and previous experience as a seasoned coordinator.
Responsibilities
- Serve as the primary point of contact for patients and caregivers - home health aides and personal care aides.
- Coordinate patient schedules and caregiver availability.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Collaborate with nursing staff as needed.
- Rotate on-call with other team members.
- Educate caregivers on specific patient needs as addressed on Plan of Care.
- Ability to multi-task.
- Capable of completing all tasks associated with filling patient requirements.
Experience
- Previous experience as home care coordinator.
- Familiarity with HHA Exchange.
- Strong organizational skills and attention to detail are crucial for managing multiple tasks efficiently.
- Excellent interpersonal skills to foster positive relationships with patients and staff alike.
- Bi-lingual a plus.

Job Type: Full-time
Pay: $25.00 - $30.00 per hour
Expected hours: 40 per week
