The Buddhist Tzu Chi Foundation, Northeast Region seeks a dedicated Licensed Masters in Social Work (LMSW) to provide comprehensive social work services with primary focus on the 1115 Waiver Initiative Project and case management services. This position requires travel throughout Queens, Manhattan, and Brooklyn to serve our diverse community members in providing charity care to underserved populations, while aligning with the Foundation’s mission of relieving suffering and improving well-being through love, respect, and action.
Key Responsibilities
1115 Waiver Initiative Project
● Coordinate and implement services under New York State's 1115 Medicaid Waiver program● Conduct comprehensive assessments to determine client eligibility and service needs ● Develop and monitor individualized service plans that align with waiver requirements● Maintain detailed documentation and reporting requirements for waiver compliance● Provide care coordination and linkage to appropriate medical and social services● Collaborate with healthcare providers, community organizations, and government agencies to enhance service delivery.
Case Management Services
● Conduct initial intake assessments and develop comprehensive service plans● Provide ongoing case management services to individuals and families● Connect clients to community resources, benefits, and support services● Monitor client progress and provide crisis intervention as needed● Advocate for clients' rights and access to services● Maintain accurate case records and participate in multidisciplinary team meetings
Community Outreach and Travel
● Conduct home visits and community-based assessments throughout NYC boroughs● Participate in community events and outreach activities● Build relationships with local community organizations and service providers● Provide education and information about available services
Administrative Duties
● Complete required documentation and reporting in timely manner● Monitor program metrics and outcomes; prepare regular reports for internal and external stakeholders.● Participate in staff meetings, training sessions, and professional development● Maintain professional boundaries and ethical standards
Required Qualifications
● Master's degree in Social Work from a CSWE-accredited program● Current New York State License for Masters in Social Work (LMSW)● Minimum 1 years of experience in case management or related social work practice● Knowledge of Medicaid 1115 Waiver Initiative and other healthcare coordination preferred● Experience working with diverse populations and cultural competency● Strong assessment, documentation, and communication skill● Proficiency in computer applications and electronic health records● Valid driver's license and reliable transportation

Job Type: Full-time
Pay: $25.00 - $28.00 per hour
