Clinical Social Worker (LCSW or LMFT)
Location: Commerce, CA
Pay Range: $48 – $50/hr
Job Type: Full-time, Temporary (potential to convert to direct hire)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM | Hybrid – On-site 1 day/week in Commerce, remote the other 4 days

Summary

The Clinical Social Worker provides behavioral health services in a primary care setting, offering assessment, diagnosis, brief therapy, crisis intervention, and discharge planning. Therapy may be delivered individually or in groups, using evidence-based approaches such as CBT, solution-focused therapy, and substance use counseling.

The role is part of a multidisciplinary team and includes collaboration with providers, case managers, and support staff to develop treatment plans and track progress. Additional duties may include contributing to program development and attending workshops or trainings.

Key Responsibilities

• Conduct psychosocial assessments and create care plans
• Deliver short-term, evidence-based therapy and crisis intervention
• Coordinate referrals with internal staff and external agencies
• Support patients and families in understanding care plans
• Maintain timely documentation in EMR (e.g., NexGen)
• Educate care team on psychosocial issues
• Contribute to program development

Qualifications

• Master’s degree in Social Work or Counseling
• Active California LCSW or LMFT license
• Bilingual English/Spanish preferred
• Minimum 2 years’ experience with children and families (internships included)
• Experience with underserved populations preferred
