A-Line Staffing is now hiring an LTSS Service Care Manager in the Naples, FL Area. The LTSS Service Care Manager would be working for a Fortune 500 company and has career growth potential. This is a full-time / 40+ hours per week position.

LTSS Service Care Manager Must Have Requirements

MUST HAVE: Experience with the Long Term Care population (Members over 65+ or the Elderly)
MUST HAVE: At least 2 years of Field Case Management Experience (Completing home visits)
MUST HAVE: Bachelor's degree in a healthcare-related field (Psychology, Sociology, etc.)
Electronic Medical Health Records experience

LTSS Service Care Manager Compensation

The pay for this position is $26 - $30 per hour based on experience PLUS mileage reimbursement
Benefits are available to full-time employees after 90 days of employment
A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

LTSS Service Care Manager Highlights

The required availability for this position is Monday – Friday, 8 AM – 5 PM
Hybrid role for Naples , FL based candidates only
80% travel required for member visits Sarasota county area
Valid driver’s license required

LTSS Service Care Manager Responsibilities

Evaluate member needs, available resources, and develop service care plans
Conduct assessments with members, caregivers, and providers for care plan development
Monitor service delivery, coordinate referrals, and follow up with members and caregivers
Ensure care plans are executed without gaps and identify functional deficiencies
Collaborate with discharge planners, physicians, and healthcare teams for proper coordination
Assist members with grievances and appeals as needed
Maintain accurate documentation within the system within 24 hours of completion
