JOB SUMMARY: Serves as the primary contact person who receives and triages calls from relative caregivers, self-referrals and community partners. Intake coordinator reviews referrals and assigns Kinship staff for program services.


ESSENTIAL FUNCTIONS:


  • Facilitates the Kinship Services Referral Line, communicating effectively, appropriately and responds in a timely fashion to phone calls, faxes and correspondences.
  • Screens referrals thoroughly within research and evaluation protocols for families that come into the Referral Line.
  • Screens and reviews referrals with Kinship Director and Program Manager.
  • Assists and supports Program Managers with contract assignments and Kinship staff.
  • Prepares and maintains electronic database, forms and memos.
  • Maintains precise and accurate records and statistics for Kinship Referral Line and Children’s Home Network programs as assigned.
  • Communicates and educates community partners about Kinship Services;
  • Collects data for research and evaluation for monitoring of program outcomes;
  • Maintains effective relationships with supervisory staff, and community resource providers, emphasizing facilitation of referrals;
  • Participates in organization’s continuous quality improvement efforts;
  • Attends training and conferences as directed by Program Manager.
  • Opens and closes cases in MyEvolv.
  • Documents contacts with caregivers in FSFN.
  • Performs other related duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS:

Education & Experience

High school graduate or equivalent from an accredited institution with 3 or more years administrative support experience in a Human Service Field. Bi-lingual, Spanish speaking required.


Licenses & Certifications:

  • Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense or license suspension.
  • Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHN’s current auto insurance policy.
  • Must be able to work flexible hours, including evenings and weekends.
  • Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check and sexual predator screening.


COMPETENCIES & PROFESSIONAL DEVELOPMENT:

Annual Training Requirements: The following training topics are required annually: Sexual Harassment, Blood-born Pathogens, Workplace Accident Prevention and Reporting, Confidentiality and Privacy Practices, Diversity/Cultural Competency, Child Abuse and Neglect Mandated Reporter Statutes and Methods, Prevention of Violence in the Workplace, OSHA–related training, Crisis Prevention, and Program Goals.


Knowledge, Skills, Abilities & Competencies:

  • Ability to communicate effectively in speech and in writing.
  • Ability to use correct grammar, spelling and punctuation in written correspondence.
  • Ability to type and accurately enter data into computerized systems.
  • Proficiency in Microsoft Office (MS Excel, MS Word, MS Outlook).
  • Possess time management and prioritization skills.
  • Possess a positive attitude concerning all aspects of working in a challenging environment, including significant patience and respect for children and families who can become quite angry and demanding, sensitivity to cultural needs and willingness to serve as a positive member of a working team.
  • Bi-lingual in Spanish required.


Supervision Received:

Supervision is provided in the form of one-on-one supervision and regular team meetings. Specific guidelines and policies and procedures form the basis for performance monitoring.

ESSENTIAL PHYSICAL SKILLS:

  • Possess good hearing (with or without hearing devices)
  • Ability to use a personal computer and related office equipment. Frequent use of hands and fingers for typing
  • Ability to speak clearly and effectively
  • Ability to work within an office setting requiring keyboard skills and administrative activities of a sedentary nature
  • Ability to operate a motor vehicle
