Supervising Care Manager

The primary role of this position will be to oversee the provision of care management. The individual in this role will be responsible for providing clinical, administrative and operations supervision and support to the Care Management Team. The supervisor will fill in for Care Managers as needed.

The outcome achieved through this position will be improved whole health of the individual served.

DUTIES AND RESPONSIBILITIES

· Responsible for providing support and direct supervision to Care Managers. One Supervisor will have a team of 8 Care Managers.

· Ensures that new and existing care managers are fully trained in care management protocols and policies, and Federal, State and local laws, rules and regulations.

· Utilizes a person-centered approach to support members.

· Utilizes operational data and reporting systems for member services and organizational initiatives as needed and appropriate.

· Participates in maintaining effective linkages in the community and the integration of all care management functions for their care managers.

· Provides support in staffing decisions.

· Manages and assigns care managers’ caseloads.

· Reviews notes and Care Plans to ensure quality and regulatory compliance.

· Provides supervisory oversight to ensure that plan goals are addressed in an appropriate timeframe.

· Monitors care managers’ productivity.

· Monitors expenses for their team.

· Fills in for care managers who may be on any type of extended leave or when there is a vacancy.

· Flexibility of work schedule is required, with 24-hour on-call coverage.

· May be required to provide transportation for individuals based on their unique needs. (ie. Doctor’s appointments, planning meetings, etc.)

· Complete and submit required monthly reports, documentation in logs, and provide incident reporting.

QUALIFICATIONS

· Must be licensed!!

. Demonstrates passion and commitment to excellence

· Meets all qualifications for Care Manager Position.

· Ability to lead an interdisciplinary care team

· Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner

· Comfortable with conducting home visits

· Possesses basic computer skills and demonstrates a willingness to learn more advanced skills

· Bi/multilingual ability preferred

EDUCATION AND EXPERIENCE

· To be considered for this job, an individual must be a master’s level licensed Clinical Social Worker (LCSW), fully licensed Clinical Mental Health Counselor (LCMHC), fully licensed Marriage and Family Therapist (LMFT), or licensure as an RN. If RN, must have current license to practice professional nursing with no restrictions.

· Minimum 3 years’ experience providing care management, case management or care coordination to the individuals with behavioral health needs.
