At Community Choices Inc. (CCI), we are dedicated to providing comprehensive substance abuse treatment and prevention services to vulnerable populations. Through our CASCADE (Courage and Support for Changing Attitudes, Dependencies, and Environments) Services Program, we support new and expecting mothers and their children in Winston Salem, NC, offering both residential and outpatient treatment services.

We are seeking an experienced and passionate Care Managers to oversee the daily operations of our North Carolina service site.

Job Summary: As a Care Manager at Community Choices, you will play a pivotal role in delivering whole-person care coordination. This role involves assessment, reassessment, care planning, and coordination of services, ensuring the development and implementation of effective, person-centered care plans. Regular communication with the member’s primary care provider (PCP) and other healthcare providers is crucial, integrating feedback from the member, caregivers, and other providers into the care plan. The ultimate goal is to improve the whole-person health of the individuals served. Your efforts will directly contribute to the improved health and well-being of our members.

Duties and Responsibilities:

- Conduct initial assessments and other relevant screening tools.

- Ensure continuity of care for newly enrolled members.

- Identify and prioritize members' needs and preferences, developing measurable goals and promoting self-management.

- Develop, implement, and monitor care plans, assisting members with obtaining necessary accommodations.

- Manage caseloads, including risk stratification and monitoring reassessment needs.

- Serve as the primary contact for members, managing their care and services, and ensuring effective communication among all parties involved.

- Lead an interdisciplinary team, facilitating activities and communication among providers, vendors, facilities, discharge planners, field nurses, social workers, and caregivers.

- Coordinate referrals to community resources and assist members with appointments, encouraging independence and responsibility for personal healthcare.

- Coordinate transitions in care, providing follow-up services within 24 hours of discharge from inpatient hospitalization or emergency departments.

- Assist with transitions from incarceration, housing changes, or other life transitions.

- Maintain accurate and timely documentation using both hard copy and technology-based solutions in compliance with CCI policies.

- Gather and summarize data for reports.

- Support Quality Assessment and Performance Improvement initiatives.

- Flexibility in work schedule is required, including 24-hour on-call coverage.

- Provide transportation for individuals based on their unique needs (e.g., doctor’s appointments, planning meetings).

- Manage a caseload with the following ratios:

- 25:1 High Acuity

- 33:1 Med Acuity

- 54:1 Low Acuity

- Complete and submit required monthly and quarterly reports, as well as incident/accident reports.

- Perform other duties as requested by the supervisor within the job's scope.

Qualifications:

- Demonstrates passion and commitment to excellence.

- Understands and can apply principles of Care Management and Person-Centered Service Planning.

- Familiar with clinical needs and disease processes for chronic physical and behavioral illnesses across diverse demographics.

- Adaptable to communication, cognitive, or other barriers.

- Ability to lead an interdisciplinary care team.

- Strong organizational skills and ability to prioritize multiple projects.

- Comfortable conducting home visits.

- Basic computer skills with a willingness to learn more advanced skills.

- Bi/multilingual ability preferred.

Education and Experience:

- Bachelor’s degree in health, psychology, sociology, social work, nursing, or a related field, or licensure as a registered nurse (RN) with a current, unrestricted license.

- Minimum 2 years’ experience working directly with individuals with behavioral health disorders.

Communication Skills:

- Strong customer service skills.

- Positive body language.

- Excellent written and verbal communication skills.

- Public speaking skills.

Mathematical Skills:

- Ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals.

Reasoning Ability:

- Demonstrates sound judgment and initiative in addressing concerns and ensuring client expectations are met.

- Ability to solve practical problems and deal with various concrete variables.

- Ability to interpret written, oral, diagram, or schedule instructions.

- Ability to read and interpret technical procedures and safety rules.

Other Skills and Abilities:

- Proficiency with fax and copy machines.

- Professional dress code (Friday dress-down day).

- Professional conduct and positive attitude.

Certificates, Licenses, Registrations:

- Initial and annual training in various areas, including First Aid/CPR, Universal Precautions, and Defensive Driving.

- Maintain a valid driver’s license, excellent driving record, and adequate auto insurance.

Physical Demands:

- Regular use of hands for typing and handling.

- Proficient in written and spoken English with the ability to hear whispers with or without a hearing aid.

- Frequent use of close and distance vision.

- Regularly required to stand, walk, sit, and occasionally climb, balance, stoop, kneel, crouch, or crawl.

- Ability to frequently lift or move up to 10 pounds, and occasionally up to 50 pounds.

Work Environment:

- Exposure to virus, disease, and infection.

- Potential for extended work periods and traumatic situations.

- Exposure to outside weather conditions.

- Noise levels range from quiet to moderate.
