Key Responsibilities:
- Conduct psychosocial assessments and develop care plans in collaboration with the interdisciplinary team.
- Assist residents and families with adjustment to the facility and support during transitional periods.
- Coordinate community resources and services to meet individual resident needs.
- Promote a positive and inclusive atmosphere that enhances residents’ well-being.
- Collaborate with the Admissions Department and participate in outreach and marketing efforts to strengthen relationships with referral sources.
- Bachelor’s degree in Social Work (BSW) or a related human services field (required).
- Prior experience working with elderly populations in long-term care or community settings (preferred).
- Strong communication, assessment, and interpersonal skills.
- Ability to work both independently and collaboratively within an interdisciplinary team.
- Experience in marketing and admissions-related functions is a plus.
