Remedy Home Health Care is looking for a compassionate and qualified Medical Social Worker to join our team. In this vital role, you'll provide expert social casework services and create social histories for patients and families receiving nursing or other care services. Your support will address challenges in social functioning that impact their health and overall well-being.

Key Responsibilities:

• Provide comprehensive social casework services tailored to patient and family needs.
• Prepare detailed social histories to guide care plans and improve outcomes.

What We're Looking For:

• Educational Excellence: Master's Degree in Social Work from an accredited school.
• Professional Experience: At least one (1) year of social work experience in a healthcare setting.
• Licensure: Registration to practice as a Medical Social Worker in the state (required).
