Compensation: $25.00–$30.00/hour (Equivalent to $52,000–$62,400/year, DOE)
Schedule: Monday–Friday (some flexibility required)

About Us:

Elevate Wellness Center is a licensed inpatient substance use disorder (SUD) treatment facility committed to providing client-centered, trauma-informed care. We are seeking a qualified and compassionate Case Manager / Counselor to join our clinical team. The ideal candidate will have experience running groups, conducting assessments, providing counseling, and maintaining accurate documentation in line with DHCS and JCAHO standards.

Key Responsibilities:

✅ Conduct intake assessments and full Biopsychosocial (BPS) evaluations
✅ Facilitate individual counseling sessions and run educational/therapeutic groups
✅ Provide ongoing case management, treatment planning, and care coordination
✅ Complete timely documentation: progress notes, treatment plans, discharge summaries, and aftercare referrals
✅ Collaborate with medical and support staff to ensure holistic client care
✅ Participate in daily clinical meetings and multidisciplinary case reviews
✅ Maintain compliance with HIPAA, 42 CFR Part 2, and all documentation standards
✅ Support clients through crisis intervention and case advocacy
✅ Uphold ethical boundaries and maintain professionalism at all times
✅ Engage in audits, training, and continuing education

Qualifications:

✔ High school diploma or GED (required)
✔ Valid SUD counselor certification (CADC-I, CADC-II, CADC-III, LAADC, CCAPP, CADTP, or CAADE)
✔ At least 1 year of experience in a licensed residential SUD program (preferred)
✔ Knowledge of treatment planning, group facilitation, and clinical documentation standards
✔ Current CPR/First Aid certification (or obtained within 30 days of hire)
✔ Must pass TB clearance and background screening as per state and facility requirements

Required Skills:

✔ Excellent group facilitation and individual counseling skills
✔ Strong documentation and treatment planning ability
✔ Understanding of co-occurring disorders, relapse prevention, and behavioral health models
✔ Cultural awareness and trauma-informed care competency
✔ Strong time management, communication, and ethical integrity

Working Environment:

• Residential facility environment
• Requires walking throughout the facility and computer-based documentation
• Exposure to emotionally intense situations; must remain calm and professional under pressure

Job Type: Full-time

Pay: $25.00 - $30.00 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
• Monday to Friday
• Weekends as needed
