Resident Care Coordinator (R.C.C.)
Job Duties and Responsibilities
- Will work in the office 3 days a week and 2 on the floor along with every other weekend, this is Mandatory.
- Help lead department by communicating with staff, being involved, and providing a great approach to medical and long-term care services.
- Must have 1 year supervisor experience.
- Must be able to communicate and follow up with physicians, planners, DON, and other Directors.
- Responsible for Scheduling care staff and Team leads, Cover shifts as needed for each position.
- Provide care to our residents which includes health/care issues and concerns.
- Able to assess vitals and report/file any changes to DON when needed.
- Create a positive and educational environment by training and coaching our staff on various topics.
- Showing of great compliance and completion of assessments, care plans, resident files, referrals, audits, state surveys, along with coordination of training and resident services.
- Participate in on-call and weekend rotation.
- Assist with coordinating move-ins, assessments, and other tasks with Marketer, DON, and Executive Director.
- Engage in care conferences with resident families.
- Med Passing experience.
- Assist with training care friends, med passers, and team leads.
- Provide a role in creating care plans based on our resident?s needs.
- Have a role in the hiring process.
- Participate in daily rounds and ongoing follow ups.
- Assist with all other duties assigned by DON and Executive Director.
Job Type: Full-time
Pay: From $18.00 per hour
Expected hours: No less than 40 per week
