The Discharge Planning Specialist is a member of a collaborative person-centered care team that provides enhanced coordination with inpatient hospitals and emergency departments. This position is responsible for solidify services, supports and documentation needed to facilitate hospital discharge and rapid release from EDs and serves as the single point of contact for discharge coordination with the RBHA and involved stakeholders.

The Discharge Planning Specialist is a member of a collaborative person-centered care team that provides enhanced coordination with inpatient hospitals and emergency departments. This position is responsible for solidify services, supports and documentation needed to facilitate hospital discharge and rapid release from EDs and serves as the single point of contact for discharge coordination with the RBHA and involved stakeholders.
MINIMUM QUALIFICATIONS
- High School Diploma / GED or Associates Degree with a combination of eight (8) years' direct care experience, required.
- Bachelor's degree in behavioral health or related field and three (3) years direct care experience, preferred.
- Previous experience in behavioral health or primary care with duties of case management and care coordination, preferred.
- Demonstrated experience and knowledge in discharge planning and readmission prevention processes, preferred.
- Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
- Must possess a valid AZ Driver's License and meet Copa's driving requirements.
Core Components of position:
- Under the oversight of the Copa Clinical Director and Director of Medical Management, implements the Copa Hospital Readmission and Discharge Coordination protocol.
- Collaborates with the Inpatient and Outpatient Care Team, assess individual needs to develop a plan with member that promotes long-term health and reduces the likelihood of re-admission. Serves as an added resource to the assigned primary case manager to facilitate individual engagement and follow-up in care, understand and implement post-discharge/post-visit medical orders and to coordinate more specialized post-discharge/post-visit placements (including skilled nursing centers, home health, and residential settings).
- Facilitate communication and coordination with the Inpatient Team, RBHA, outpatient team-centered team, and family and/ or natural supports, in order to develop and implement the discharge plan.
- Completes all necessary discharge planning documents and submits documentation and referral packages in a timely manner to the RBHA and community providers.
- Coordinates with the Copa care team to ensure scheduling of post-discharge appointments within 72 hours of the discharge. Ensures follow-through with discharge recommendations and services, and identifies gaps in services
- Ensures the care team implements necessary post-discharge services, including medication re-fills, transportation and needed follow up supports, including weekend and after hour visits.
- Facilitates coordination and tracking of service referrals identified on the individual's discharge plan.
- Travel as required to attend discharge staffing, appointments, to obtain vital records and to attend community-based appointments as needed.
- Serve as the primary responsible contact for CPRP Notifications and Hospital admissions/discharge notifications and alerts through the health information network.
- Remain available to answer any questions or provide support as necessary after member discharge.

- Complete all required documentation in a timely manner consistent with company guidelines.
- Manage and complete assigned work tasks by deadlines.
- Participate in regular staff meetings, required training programs, clinical supervision, etc.
- May be required to be part of an on-call rotation after hours.
- Other duties as assigned.
