Job Title: Licensed Master Social Worker (LMSW)
Location: New York, NY 10016
Pay Rate: $2,062.50 – $2,250.00 per week
Schedule:

  • Hours per Day: 8

  • Days per Week: 5

  • Total Hours per Week: 40

  • Shift Time: 9:00 AM – 5:00 PM

Key Responsibilities:

  • Perform psychosocial assessments and develop individualized care plans

  • Support discharge planning by coordinating with community resources and social services

  • Deliver crisis intervention, emotional support, and short-term counseling

  • Work collaboratively with physicians, nurses, case managers, and allied health professionals

  • Advocate for patients and assist in addressing social, emotional, and financial barriers to care

  • Maintain thorough and timely documentation in electronic medical records

  • Participate in interdisciplinary rounds and case conferences

Qualifications:

  • Active LMSW license issued by the New York State Education Department (NYSED) (primary source verification required)

  • Minimum 1 year of experience practicing as an LMSW

  • Recent hospital or inpatient experience strongly preferred

  • Proficient in discharge planning, community referrals, and social service navigation

  • Excellent interpersonal, counseling, and problem-solving skills

  • Ability to function both independently and within a multidisciplinary care team
