For over 60 years PSS has performed vital and important work – through its 10 senior centers, caregiver services, and affiliated programs, the agency touches the lives of over 2,000 people a day. The Social Worker Case Manager will be a part of the Case Management team, under the Social Work Supervisor. The Social Worker Case Manager will provide robust case management services, the necessary referrals and community connections, as well as supportive counseling to older adults, so that they may age in place within their homes in the community. Additionally, the Social Worker Case Manager will draft care plans, conduct home visits, and administer assessments, to ensure the needs are met for all assigned older adults.
RESPONSIBILITIES:
- Perform in-home assessments and reassessments of homebound older adults and develop comprehensive care plans
- Assess the formal and informal supports of each older adult in the community and begin to develop relationships with them;
- Provide extensive and ongoing case management services including information, case assistance, entitlement counseling, referrals, supportive contact, advocacy, and crisis intervention;
- Keep detailed and comprehensive records of all contact with clients, including case files and unit entries in the database;
- Initiate referrals to and coordinate activities with homecare agencies and homebound meal delivery services;
- Ensure compliance with all programmatic requirements; including timely client contacts, assessments, and home visits;
- Attend trainings as required including mandatory NYC Aging trainings;
- Perform related duties as required by the Case Management Supervisor, such as visiting seniors in their homes.
QUALIFICATIONS AND EXPERIENCE:
- Bachelor's degree in social work (BSW or LMSW desired)
- At least 3 years of social work experience in working with the older adult population
- Bilingual in English/Spanish
- Strong interpersonal, clinical, organizational, and presentation skills
- Familiarity with benefit, entitlement, and other resource applications and re-certification procedures
- Experience and familiarity with performing home visits and engaging older adults in their environments
- A passion and sensitivity for working with older adults
