Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

Position Overview:

The Social Worker on the APU is responsible for the coordination of social service delivery to patients and families, as well discharge planning including coordination of follow up care. Areas of responsibility include discharge planning, psychoeducation and counseling for patients and families, and assisting team with implementation of comprehensive treatment plans.

Essential Functions:

  • Work closely with Director of Social Services, Program Director, Psychiatric Medical Director and Assistant Medical Director, Nurse Manager, and all clinical staff to meet the psychosocial needs of patients and families.
  • Provides individual and group counseling, communicates with families and the patient’s authorized representatives, conservators, physicians, and other health team members.
  • Coordinates discharge planning assists with advance directives, financial and legal issues and maintains documentation of services in the medical record.
  • Participates in the development and implementation of the comprehensive treatment plan.
  • Provides group and individual counseling as needed.
  • Reports any claim of elder/dependent abuse promptly per facility policy.
  • Attends and participates in IDT meetings with physicians and staff.
  • Provides utilization assessments.
  • Assists physician with completion of legal paperwork/documentation, mindful of individual county time constraints.
  • Assists patients with coordination of complaint/grievance/compliment process. Logs all complaints/grievances/compliments per facility policy. Communicates resolution with person submitting grievance per facility policy. Coordinates this process with the Program Director.
  • Maintain current licensing status and complete required continuing education.
  • Provides crisis intervention, family-focused treatment planning and conflict resolution services as needed.
  • Complete timely assessments and treatment planning.
  • Attends house-wide in-services per facility policy.
  • Performs other duties as assigned.

Requirements:

  • MSW Required.
  • Two years’ experience working with psychiatric patients preferred.
  • Two years’ experience in social services and discharge planning knowledge and experience in geriatric setting, preferred.
  • Excellent interpersonal and communication skills.
  • Knowledge of MS Office, specifically Word and Outlook.
  • Knowledge of social service agencies, state regulations, and professional board standards as related to patient rights and confidentiality.
