Assisted Living LPN Care Manager
Reports to: Director of Administrative Services / Executive Director
License Required: LPN
Community: All Assisted Living Communities
Hours per Week: Hourly Position, 40 Hours Weekly
Wage: $31.00 per Hour
This LPN position is a full-time leadership position shared between our three assisted living communities: The Washington Center, Trinity Terrace, and Monarch Meadows. This position works with the Director of Administrative Services, Executive Director, and assists our RN Care Manager and Resident Service Directors in providing daily clinical oversight and support for residents and resident assistant staff, using shared decision-making, interdisciplinary teamwork and collaboration resulting in positive resident and staff outcomes. This is a fast-paced position that requires commitment, proficient organizational and computer skills, and excellent communication skills. This position requires daily face-to-face interaction with staff, residents, and family members, problem solving, following up on resident care, updating care plans, and educating and working with staff on problem solving and skill development.
Primary duties include:
· Work with each assisted living’s Resident Service Director providing support and guidance for resident care.
· Interview residents, complete required care evaluations, and develop individualized service plans for each resident upon admission, semi-annually, and with change of condition.
· Read daily notes to review residents for order changes and change of condition and follow up as needed.
· Follow up with caregivers, residents, and family members regarding order changes and change of condition.
· Review orders daily and update the EMAR system.
· Regularly review vitals, weights, and labs.
· Draw blood; give vaccinations, do covid tests, perform treatments not delegated to resident assistants.
· Add diagnosis codes to the electronic health record.
· Conduct unit rounds at each community weekly.
· Visit residents as needed to evaluate resident care concerns.
· Perform medication delegation staff assessments.
· Participate in teaching and training of resident assistants.
· Quality Improvement med-cart audits.
· Communicate in person, via fax, telephone, etc. with medical providers. Follow up on concerns until resolved.
· Review and enter orders for new admissions.
· Participate in creation and review of clinical policy and procedures.
· Review falls and incident reports for appropriate intervention.
· One week on-call rotation every 4 weeks. Will be paid a daily stipend of $35.00 for each day.
· Work as a Resident Assistant during on-call week if necessary. Will be paid regular hourly rate for any hours worked as a Resident Assistant during on-call week

Scheduled Hour Times:
Full-time day shift hourly position, typically works Monday through Friday from 8:00 a.m. – 4:30 p.m. Leadership team members are required to maintain a work schedule that ensures support for community services and your coworkers.
Job Requirements
- Must be a graduate of an accredited School of Nursing
- Must possess a current, unencumbered license to practice as a LPN in the State of WI
- Must project a positive image of the facility by displaying a high degree of professionalism, demonstrating concern and courtesy in interactions with residents, families, medical staff, employees, volunteers and outside agencies affiliated with the facility
- Must demonstrate the ability to function independently, have personal integrity, flexibility, and the ability to work effectively with residents, personnel and support agencies, and to make independent decisions when necessary
- Must be able to communicate effectively and clearly.
Preferred Experience / Knowledge
- Experience in nursing specialty of Geriatrics or Long-Term Care
- Familiarity with State and Federal Regulations of SNF and CBRF, or willingness to pursue further training in these areas.
