Position Summary:
We are seeking a compassionate and dedicated Housing Case Manager to support families in their journey toward housing stability and self-sufficiency. The Housing Case Manager will coordinate services for families by developing personalized action plans, connecting clients to community resources, and providing follow-up support during their transition into permanent housing.

Key Responsibilities:

  • Manage a caseload of 30 families
  • Conduct twice-monthly meetings with program participants to develop and review individualized action plans for housing stability.
  • Assess clients’ needs, including work readiness, and make referrals to workforce development, employment opportunities, and other community resources.
  • Establish relationships with potential employers and provide employment leads.
  • Maintain relationships with referral agencies to connect clients to mental health, medical, and other supportive services.
  • Assist clients with accessing rental and utility assistance within program budgets.
  • Ensure accurate documentation of case notes, disability documentation, and service delivery in HMIS and other data systems.
  • Submit assistance payments promptly and maintain up-to-date client files.
  • Collaborate with team members and community partners to support client success and program goals.
  • Provide feedback to the Housing Program Director and participate in team meetings, trainings, and supervision.
  • Uphold confidentiality and maintain professionalism at all times.
  • Respond effectively to emergencies and ensure compliance with all policies and procedures.

Qualifications:

  • Bachelor’s degree in social work, education, psychology, sociology, or related field.
  • LSW license in Ohio is a plus.
  • 1-2 years experience working with families experiencing homelessness or at risk.
  • Strong understanding of trauma, cultural sensitivity, and socioeconomic factors affecting clients.
  • Ability to provide positive role modeling and maintain appropriate boundaries.
  • Proficiency with Office 365 and reporting software.
  • Valid driver’s license and ability to meet vehicle insurance requirements.
  • Reliable attendance and ability to work flexible hours as needed.

Physical & Environmental Requirements:

  • Ability to perform physical activities such as kneeling, standing, lifting up to 10 pounds, and operating a motor vehicle.
  • Capable of working in various environments including client homes, community settings, and offices.
  • Extended use of a computer required.
