Responsibilities and Duties

BRIEF JOB DESCRIPTION BELOW:

• Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the Interdisciplinary team.
• Completes psychosocial evaluations, plans interventions, and documents in a timely fashion.
• Provides counseling and crisis intervention as well as grief and bereavement follow up.
• Participates with all parties in the development of the plan of care, case conferences as required, Election of Benefits, discharge planning and other hospice related procedures.
• Provides hospice related education, information and referrals to community services to support patients and families/caregivers hospice needs.
• In some cases, serves as liaison between patients or families/caregivers and community agencies.
• Maintains good relations with Advantage HS contracted agencies.
• Requires on call hours, scheduled rotation among six other clinical staff.

Qualifications and Skills

QUALIFICATIONS:

• Graduate of a Master’s program in Social Work accredited by the Council on Social Work Education.
• Minimum of one year of social work experience in health care setting.
• Experience in a home health care preferred.
• Demonstrates excellence in the following: team work; verbal and written communication skills; and excellent time management and organization skills.
• Possesses or is willing to obtain and maintain CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Job Type: Part-time

Pay: $30.00 - $40.00 per hour
