POSITION SUMMARY:
The Bereavement Coordinator Social Worker provides comprehensive psychosocial services to patients and families, supporting them through the complexities of illness, care management, and end-of-life issues. This role involves performing psychosocial, home, family, and financial assessments for community-dwelling patients, as well as evaluating risk factors such as abuse, neglect, and suicide. The Medical Social Worker advocates for patients by identifying appropriate community resources, facilitating referrals, and assisting with long-term care planning, placement, and financial counseling.
In collaboration with physicians, nurses, and the interdisciplinary care team, the Medical Social Worker develops and implements individualized care plans that address the psychosocial, emotional, and bereavement needs of patients and their families. The role includes ongoing supervision of Licensed Social Workers, oversight of bereavement care, including risk assessments, volunteer support, and grief support groups-and participation in interdisciplinary team meetings.
The Medical Social Worker maintains compliance with regulatory standards, participates in quality improvement initiatives, and supports staff development through education and training. Additionally, the position requires the development of strong referral relationships, community outreach, and planning of memorial services, all while ensuring the highest level of compassionate, patient-centered care.
MINIMUM QUALIFICATIONS:
Education: Masters degree in Social Work
Required length and type of experience: Minimum 1-years experience in social work working in healthcare setting required.
Required licensure, certification or registry: Licensed as an LSW in the state of OhioYou must possess a valid driver's license and an acceptable driving record. Must be insurable under professional liability and crime
