Job Details
Act as a liaison among family, school, and social agencies to obtain those services needed to support their children in a school setting.
Job Requirements
Provides individual and group counseling, crisis intervention, and/or mediation services.
Acts as liaison between family and school.
Provides information to parents and caregivers regarding student progress and when directing them toward resources and interventions available in the community.
Conducts home assessments and/or visits as deemed necessary with the approval of the principal.
Consistently communicates and consults with parents, school staff, and outside agencies and supports to both communicate student progress and collect relevant feedback
Develops, communicates, implements, monitors, and adjusts student transition plans integrating clinical supports, academic supports, care coordination, and family engagement components of the overall case management plan for each student.
Complete other duties as assigned by the building administration
