What is PACE North?

PACE stands for Program of All-Inclusive Care for the Elderly. We are a trusted option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly daily both at our state-of-the-art day center on Garfield near Hammond during regular business hours Monday through Friday-and as needed in their homes.

What does a Home Care Coordinator do at PACE North?
  • Conducts initial and periodic assessments of participants' home care and safety needs.
  • Collaborates with the interdisciplinary team (IDT) to develop and revise individualized care plans.
  • Coordinates care in participant homes, nursing homes, assisted living, and adult foster care settings.
  • Communicates with participants, families, and home care staff to ensure needs are met and concerns are addressed.
  • Tracks and manages home care supplies and services such as durable medical equipment, wound care, and incontinence products.
  • Maintains accurate and timely documentation using electronic medical records.
  • Participates in quality improvement initiatives and audits.
  • Ensures participant rights are respected and upheld.
  • Drives to participant homes and community facilities as needed.

Please note: This is a full-time, in-person opportunity based in Traverse City, Michigan. Home visits and occasional off-schedule or weekend work may be required based on participant care plans.

Requirements

What qualifications should you bring to the table?
  • Education: High School Diploma or equivalent required; Associate degree or college coursework preferred.
  • Licensure/Certification: Valid CPR certification and Michigan Driver License with current auto insurance and acceptable driving record.
  • Experience:
  • At least 1 year of experience working with frail or elderly population preferred.
