TOUR 2 (12am-8pm)

PUPROSE OF POSITION:

Under supervision, may supervise social work staff as assigned. Provides social work services and the coordination of continuing care planning, utilizing a variety of techniques such as individual, family, group work, and community networking to patients serviced at this facility

AREAS OF RESPONSIBILITIES:

SUD/ ED

SUMMARY OF DUTIES AND RESPONSIBILITIES:

• Reviews the medical records of patients in order to identify those whom will require SUD intervention and counseling

• Initiates pt.’s referrals to appropriate in house or community-based SUD programs.
• Completes Social Work Assessment for these high/moderate-risk patients as needed.
• Maintains effective communication and provides supportive counseling, crisis intervention to the patient with regards to addiction treatment options and services
• Collaborates as a liaison with Chemical Dependency department staff/programs
• Documents all Social Work interventions in the patient’s electronic medical record in accordance with departmental and hospital policies.
• Maintains effective communication with interdisciplinary team and participates actively of interdisciplinary discharge planning rounds and records plans/recommendations in the patient’s medical record.
• Provides shadowing for new ED Leads staff and coverage to CATCH when designated. Acts as a resource for SW staff.
• Assist in the orientation and training of new staff of the SW department and other disciplines as needed.
• Will provide individual and group therapy to patients receiving services through Chemical dependency department as needed.
• Complies with requirements for supervision, training, Performance Improvements Projects, Quality Assurance and Research Studies.
• Understands and follows the escalation process as needed.

For Assignment to Level II

1. Valid license as an LMSW issued by the NYSED;

or 2. Valid license as an LCSW issued by the NYSED.
