Position Summary: We are seeking a dedicated Health Home Care Coordinator to join our team. The Care Coordinator plays a key role in developing, implementing, and monitoring individualized care plans for clients. This position involves working collaboratively with health care providers, social services, and community agencies to ensure holistic and effective support for our clients.
Key Responsibilities:
- Conduct comprehensive assessments to identify physical, behavioral, and social service needs.
- Develop and maintain individualized care plans in collaboration with the client and interdisciplinary team.
- Coordinate services such as primary care, mental health, substance use treatment, housing, and social supports.
- Monitor client progress through regular follow-up and care plan reviews.
- Maintain accurate and timely documentation in compliance with Health Home program requirements.
- Advocate for clients and facilitate access to resources that support health and well-being.
- Collaborate with hospitals, clinics, and community partners to ensure continuity of care.
Qualifications:
-Must have HH+ experience
-Must have previous experience in Care Management role
- A Bachelor’s degree in Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational Therapy, Physical Therapy, Recreation Therapy, Counseling, Community Mental Health, Child and Family Studies, Sociology, Speech and Hearing, or another Human Services related field OR New York State Registered Nurse (RN) license and registration with a bachelor’s degree OR Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
- Minimum of 3 years of experience in care management or a related healthcare field.
- Strong knowledge of healthcare regulations, care coordination, and case management practices.
- Ability to manage multiple priorities and work effectively in a fast-paced environment.
- Proficiency in using care management software and Microsoft Office Suite.
- Commitment to providing high quality care and improving client outcomes.
-Experience Providing direct services to people with physical chronical conditions, serious mental illness, developmental disabilities, or substance use disorders
or helping people with these conditions connect to a wide range of services to support them in the community
Job Types: Full-time, Part-time
Pay: $20.00 - $25.00 per hour
