As a Social Work Care Manager at Hoag Memorial Hospital in Newport Beach, you will collaborate with medical staff and the community to ensure extraordinary patient care. Your responsibilities include coordinating post-acute services based on clinical assessments, providing psychosocial support, grief counseling, and psychiatric stabilization.

Inside the Care Management Department

Our team ensures streamlined coordination of patient care from admission to post-discharge, emphasizing appropriate, timely, and cost-effective care. We value excellent communication, teamwork, effective listening, and customer service skills.

Other Duties Include:

  • Assessing discharge needs with an interdisciplinary team.
  • Developing comprehensive discharge plans.
  • Interviewing patients to identify barriers to discharge.
  • Providing psychodynamic intervention, crisis counseling, and end-of-life support.
  • Adhering to HIPAA guidelines and coordinating community resources.
  • Documenting patient issues, discharge plans, and contact information.
  • Educating patients and families about healthcare options and resources.
  • Advocating for patients to receive appropriate services.
  • Utilizing resources during and after hospitalization.
  • Participating in hospital quality metrics and initiatives.
  • Understanding payer requirements and mandated reporting responsibilities.
  • Completing necessary competencies as applicable.

Minimum Requirements:

  • Masters Degree in Social Work (MSW).
