Hospice Social Worker (MSW) - Lily Hospice About Lily Hospice:

Position Summary: Lily Hospice is seeking a highly motivated and empathetic Master's-prepared Social Worker (MSW) to join our interdisciplinary team. The Hospice Social Worker will provide comprehensive psychosocial assessments, counseling, and resource coordination to patients and their families, ensuring their emotional, social, and practical needs are met throughout the hospice experience. This role requires a strong understanding of end-of-life care, grief and bereavement, and community resources. This position is for Ozaukee, Washington and Sheboygan counties.
Responsibilities:
* Psychosocial Assessments: Conduct thorough psychosocial assessments to identify patient and family needs, including emotional, social, financial, and spiritual concerns.
* Counseling and Support: Provide individual, family, and group counseling to address grief, loss, anxiety, and other psychosocial issues related to the patient's illness and prognosis.
* Care Planning: Develop and implement individualized care plans based on assessments, collaborating with the interdisciplinary team to ensure holistic care.
* Resource Coordination: Connect patients and families with community resources, including financial assistance, respite care, support groups, and bereavement services.
* Crisis Intervention: Respond to and manage crisis situations, providing immediate support and intervention as needed.
* Advance Care Planning: Facilitate discussions about advance directives, living wills, and power of attorney, ensuring patient wishes are documented and respected.
* Bereavement Support: Provide bereavement counseling and support to families following the patient's death, offering resources and guidance during the grieving process.
* Documentation: Maintain accurate and timely documentation of assessments, interventions, and care plans in accordance with hospice policies and regulations.
* Interdisciplinary Team Collaboration: Participate actively in interdisciplinary team meetings, contributing to the development and implementation of comprehensive care plans.
* Compliance: Adhere to all federal, state, and local regulations, as well as hospice policies and procedures.
* Community Outreach: Participate in community outreach and education activities to promote hospice services and raise awareness about end-of-life care.
Qualifications and Skills:
*Master’s degree in Social Work from an accredited institution
*Experience working in hospice or palliative care settings is preferred
*Proficient in crisis intervention techniques
*Strong understanding of end-of-life issues and grief processes
*Exceptional communication and interpersonal skills
*Ability to work collaboratively within an interdisciplinary team
*Familiarity with community resources for terminally ill patients
*Experience in family counseling and support
*Knowledge of healthcare laws and patient rights
*Strong organizational and time-management skills
*Commitment to providing compassionate care to patients and their families
