Are you a motivated RN or licensed Social Worker ready to make a difference in hospital care coordination? We're seeking a full-time Case Manager to join our dynamic team in North Central New Mexico. This role is ideal for professionals who excel at managing care transitions, advocating for patients, and improving outcomes across the care continuum.

Key Responsibilities:
  • Conduct patient assessments and create tailored care plans from admission through discharge
  • Ensure patients receive appropriate post-acute care, including home health, hospice, or skilled nursing
  • Serve as a key liaison between patients, families, providers, and insurance representatives
  • Help reduce hospital readmissions and improve length of stay efficiency
  • Identify and address barriers to care with strategic problem-solving
  • Complete timely documentation and deliver required patient notices (e.g., Medicare Important Message)
  • Collaborate with medical teams to ensure compliance with regulatory and payer guidelines


Required Qualifications:

Education:
  • RN: Graduate of accredited Nursing program (BSN preferred)
  • OR Social Work: MSW from accredited institution

Licensure & Certifications:
  • Active NM RN license OR valid LMSW, LISW, or LCSW license
  • BLS certification (American Heart Association)
  • Case Management Certification (CCM, ACM, or RN-BC) preferred

Experience:
  • Minimum 2 years in healthcare setting
  • Prior experience in acute care or hospital case management strongly preferred

Skills & Competencies:
  • Strong communication, advocacy, and collaboration abilities
  • Knowledge of discharge planning, insurance protocols, and community resources
  • Proficient with EMR systems and documentation best practices
