The LTSS Service team works together with individuals and their person-centered planning team to identify, coordinate, and facilitate all necessary services regardless of the funding source.If you are passionate about your career, enjoy helping people, and want to make a difference in the lives of others, apply now!Summary of Responsibilities:Under the general direction of the Director of Long Term Support Services, this person manages a portion of the daily operations of the Service Coordination unit Long Term Support Services (LTSS) waiver service and Community Health Choices, eligible individuals. This person provides supervision for staff to ensure that eligibility criteria for services are achieved. Duties are conducted in accordance with all existing policies, procedures, and regulations and in accord with the Mission and Values of the Company.Essential Functions:
  • Ensures plans are developed and completed
  • Ensures individuals are receiving appropriate monitoring
  • Ensures waiver-funded individuals meet Medical Assistance requirements
  • Ensures that employees adhere to billing requirements
  • Ensures that the Incident Management process is followed
  • Ensures that adequate follow-up is performed
  • Prepares reports
  • Completes required trainings
  • Maintains cooperative and professional relationships
  • Serves as a link between subordinates and upper management
  • Provides supervision and oversight to all unit staff
  • Monitors Service Coordinator productivity
  • Provides supervisory and caseload coverage as needed
  • Ensures that Individualized Support plans, Comprehensive Needs Assessments and Person Centered Plans process is followed
Personal Characteristics:Independent, Tech-Savvy, Responsive, Kind, Ethical, Motivated, Active Listener, Life-Long Learner, Adaptable, Confidential, Resourceful, Values, & TrustworthyEducation and Experience:RN or a Pennsylvania-licensed social worker or Pennsylvania-licensed mental health professional with at least three (3) years of relevant experience. Experience in a supervisory, managerial, or leadership role, preferredEmployment Requirements:Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening ClearanceWorking Conditions:Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, work may be required at night and on weekends, travel throughout the SAM footprint may be extensive and may be in adverse weather conditions, overnight travel will be required, must be able to lift up to 15 pounds.
