Supervisory Responsibilities
Carries out supervisory responsibilities as designated by the Vice President of Quality and in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

- Provides leadership and oversight of the daily work of St. Anthony's Social Services department, including coordination of regular Social Services meetings.
- Coordinates staffing and cross coverage for social work needs across the organization.
- Organizes and oversees training of all Social Services staff, including onboarding education and coordination of continuing education opportunities.
- Develops and maintains Social Services policies and procedures for the organization.
- Assists the Vice President of Quality in completion of annual staff evaluations.
- Implements and coordinates assigned Social Services projects for the organization, including quality assurance and performance improvement projects.
- Facilitates the discharge planning process by coordinating care between the hospital and with other agencies, community support systems, home health care, and equipment suppliers to promote continuity of care and assure timeliness of care.
- Provides emotional support, counseling and intervention to patients/families, residents and employees facing change situations, and assists in referrals when appropriate.
- Guides and assists providers in discharge and placement of patients, coordination of care of outpatients, and alignment of services for patients in all areas of the organization.
- Coordinates admissions to skilled care by providing information and understanding to patients/families to this level of care.
- Functions as a team member in patient care conferences and other community service programs conferences.
- Completes psychosocial assessments and documentation as required in various departments across the organization.
- As appropriate, administers a depression scale, recommends interventions, and coordinates resources for treatment.
- Assists in completion of PASSR documentation as needed.
- Promotes rights of patients through advocacy.
- Oversees the development, implementation and facilitation of new programs, groups and events related to social services in conjunction with social workers.
- Works willingly and accepts assignments as given.
- Attends all mandatory meetings and programs.
- Performs other duties as necessary or assigned.
- Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.
- Complies with all applicable state and federal regulations and JCAHO standards.
- Maintains a safe work environment using safe work practices, utilizing standard precautions and complying
with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.
- Maintains the confidentiality of patient, resident, employee and organizational information.

Education and/or Experience
- Master's Degree in Social Work.
- Holds a current state license as a Social Worker.
- Experience in a health care setting preferred.

- Maintain required hours of CEU's for Master's Degree in Social Work.
- Reliable transportation, current valid Iowa driver's license and proof of automobile insurance coverage.
- Good interpersonal and written and verbal communication skills.
- Must have demonstrated ability to work cooperatively as a team member and provide consistent direction and leadership.
- Physical, sensory, and cognitive abilities sufficient to perform essential functions.
