Title: Care Coordinator / LPN

Worksite: Haven 4 Change

County: Warren

Hours Status: Full-Time, M-F, 8a-5p

Pay Range: $22.60 hourly

POSITION SUMMARY:

Advocate for and assisting clients experiencing chronic substance misuse with their health care needs. Provide medical oversight in transitional living setting including managing medication logs.

Provide care coordination services to women and her children in transitional living setting in order to maintain a continuum of care while in treatment and during transition between ASAM levels of care for both women and her children, focusing on pregnant and parenting women with opioid use disorder. Involves direct, written, and telephonic contacts with clients and their service providers to improve outcomes and to ensure access to care and needed supports.

QUALIFICATIONS:

• Must be a licensed practical nurse.

• Maintain a valid Driver's license and automobile insurance coverage.
• Availability during non-work hours for on-call consultation.

SKILLS:

• Work experience in a medical office setting preferred.
• Experience providing case management or care coordination or working with clients with substance use disorders is strongly preferred.
