JOB SUMMARY: The Mental Health Therapist is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care, and chronic disease management patients to effectively provide linkage and advocacy to fulfill clients' needs. Estimated volume may be around 2,850.

***Per the client, would like a candidate who has actually been seeing patients or teaching (not in a leadership position), some clinical experience with all ages (or at least the ability and willingness to work with all ages, experience with an integrated model (partnering with referring Physicians). 1-2 years of experience in a behavioral health setting - Required
Master's degree from an accredited institution in Social Work, Counseling, or related field; or a Doctorate degree in clinical psychology. - Required
Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Practitioner (LMHP, LIMHP), or Licensed Psychologist. - Required
Effective communication skills
Exceptional customer service skills
Experience with electronic medical records
Provides patient care for all ages
