If you seek a position with purpose, where you can share your clinical expertise and face challenges, the Department of Community and Human Services (DCHS) is hiring a Part-Time Senior Therapist, State Hospital Discharge Planner. This role involves independent planning for hospitalized clients discharged to the community and assessing their community service needs. The position works closely with the full-time discharge planner to ensure all discharge planning needs are met.
The Opportunity
- Provides discharge planning and aftercare services following Virginia protocols and guidelines;
- Assesses aftercare needs of clients hospitalized in state mental health facilities;
- Identifies and explores relevant community resources;
- Links clients to appropriate services;
- Provides follow-up case management to ensure successful community transition;
- Makes referrals and advocates for timely service provision;
- Prepares reports as required;
- May supervise graduate interns;
- Provides individual, group, family therapy, and case management;
- Manages administrative tasks related to the Discharge Assistance Program (DAP), including contracts and invoices;
- Ensures proper service delivery from private vendors;
- Attends meetings at hospitals, community settings, and clients homes;
- Conducts assessments, evaluations, and formulates diagnoses;
- Provides emergency and crisis interventions;
- May prepare court reports, testify, or serve as the Not Guilty by Reason of Insanity Coordinator;
- Represents the CSB at regional meetings;
- Performs other duties as assigned.
What We Are Looking For
The ideal candidate will have thorough knowledge of individual and group behavior, psychiatric issues, dual diagnosis, chemical dependency, psychopharmacology, and related fields. They should be able to work successfully with diverse consumers, communicate effectively, and supervise interns or students.

Minimum & Additional Requirements
- Masters degree in Social Work, Mental Health Counseling, Psychology, Education, or related field;
- At least two years of post-graduate clinical experience with consumers with substance abuse, psychiatric, or emotional disorders;
- Licensure or license eligibility as LPC, LCSW, LSATP, or Clinical Psychologist in Virginia, with licensure obtained within one year if eligible;
- A valid drivers license is required if driving responsibilities are involved.
Preferred Qualifications
- Fluent and bilingual in English and Spanish;
- At least one year of discharge planning experience with individuals with serious mental illness;
- Experience linking consumers with community resources in community mental health or psychiatric hospital settings.
