The Behavioral Health Case Manager provides behavioral health case management and behavioral health rehabilitation services. This position requires population specific competencies. Successful application to and mandated required training by the Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS). Responsibilities

The behavioral health case manager responsibilities include, but are not limited to, the following:
  • Provides a variety of therapeutic services to include, but not limited to, rehabilitation group therapy and case management.
  • Conducts pre-screening telephonic interviews to determine candidacy of patient for intensive residential treatment.
  • Facilitates the scheduling of on-site intake assessments with medical and behavioral health team.
  • May assist licensed behavioral health professionals with family and patient screening using standardized assessments.
  • Participates as a member of interdisciplinary team and team conferences.
  • Acts as a liaison between patients, family members, physicians, interdisciplinary team and external agencies and organizations making referrals as appropriate.
  • Facilitates meetings for discussion of plan of care, community resources and discharge planning.
  • Conducts follow-up telephone interviews at 3, 6, 9, and 12-month intervals using standardized assessment tools.
  • Documents group therapeutic activities in electronic medical record.
  • Consults with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
  • Develops and maintains positive relationships with community partners; facilitates contact with outside referral sources to confirm receipt of referrals.
  • Schedules and coordinates psychological testing, community resources, and vocational referrals, as needed.
  • Works with adult patients only.
  • Will not supervise staff.

The Behavioral Health Case Manager reports to the Director of Clinical Services Mental Health staff must be able to assist with physical restraint of patients, utilizing the CAPE System.

This position may have additional or varied physical demand and/or respiratory fit test requirements. Please consult the Physical Demands Project SharePoint site or contact Risk Management/Employee Health for additional information. Potential for exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals, and physical danger from disturbed/irate patients. Must follow standard precautions.

Qualifications

•Have a minimum of thirty-six (36) months of direct, documented experience working with persons with mental illness and/or substance use disorder and possess a High School Diploma, General Equivalency Diploma (GED), or High School Equivalency (HSE) Credential; or
•Have completed sixty (60) college credit hours and have a minimum of twelve (12) months of direct, documented experience working with persons with mental illness and/or substance use disorder; or
•Have a Bachelor's or Master's degree in any field earned from a regionally accredited college or university recognized by the United States Department of Education (USDE) and have a minimum of six (6) months of direct, documented experience working with persons with mental illness and/or substance use disorder; or
•Have a Bachelor's or Master's degree in a behavioral health related field earned from a regionally accredited college or university recognized by the United States Department of Education (USDE); or
•Have a current license as a registered nurse in the State of Oklahoma with documented experience in behavioral health care.
