Currently we are seeking a Social Worker for our program within our Womens Health outpatient practice, in Niagara Falls, NY!
The primary purpose of this position is to provide patients/caregivers with supportive counseling, psychosocial assessments, therapeutic interventions, education, and community resource support information.
This is a Monday through Friday position, scheduled from 8am- 4pm. Hybrid opportunity!

Essential Duties:
- Provides psychosocial support and crisis management for patients throughout the continuum of care, including but not limited to diagnosis, living with chronic disease, recurrence, end-of-life, and survivorship.
- Participates in multidisciplinary program development and evaluation, treatment modalities and emergency crisis/response.
- Acts as a liaison for appropriate referrals to additional services including but not limited to psychiatry, psychology, supportive and palliative care, pain management, spiritual care, patient navigation and patient, family, and community education.
- Collaborates with and promotes teamwork among all member of the healthcare team and openly expresses ideas for new programs or enhancements to current programs and services.
- Utilize social work theory, principles, and methods to provide strength-based assessment, evaluation, formulation, and implementation of a treatment plan to address the psychosocial aspects of illness and its impact of the patient and caregivers.
- Participates in/ assists with the planning and implementation of performance improvement activities to promote continuous improvement in care delivery.
- Assists in education and training programs, policy writing, and research/program development.
- Deliver exemplary customer service to provide a positive patient experience across the organization.
- Perform other duties assigned by management
Pay Range: $65,000-$85,000
Minimum Qualifications:
- Master's Degree in Social Work (MSW) and must have NYS licensure as LCSW-R or LMHC
- Two years Clinical Social Work required
