SERVICE COORDINATOR
Youthful Offender Wraparound - Irvine. F/T, M – F, 9 a.m. – 6 p.m. (Must be available for after-hours emergencies with 24/7 capability to respond via agency issued cell phone). Hybrid Available.
Responsibilities Include
- Provide in-home mental health rehabilitation services to youth who are involved in the Juvenile Justice system.
- Assist consumers, ages 12-15, with daily activities related to their Care Plan to become inter-dependent, self-reliant and successful.
- Lead the implementation of a service plan covering the entire range of needs for the family to reduce recidivism and promote success, safety, wellness, recovery and permanence in the home, school, workforce and community.
- This role works with the family to increase positive influence and decrease negative behavioral issues.
- Maintain a “whatever it takes” approach to resolving challenges and assisting families to focus on meeting their goals.
- Daily documentation of any service provided.
- Bachelor’s Degree in psychology, social work, criminology or related discipline.
- 1-2 years’ experience in the field.
- Establish rapport and make accurate assessment of a family's needs.
- Assist families in accessing and utilizing resources provided.
- Excellent written and communication skills.
- Ability to work with a multicultural and diverse population with sensitivity and respect.
- Must be available for after-hours emergencies with 24/7 capability to respond via agency issued cell phone.
- Bilingual English/Spanish required.
