JOB SUMMARY


The Care Coordinator addresses the various social issues of our patients with the goal of promoting optimal health. Using the process if interviewing, planning and implementation of care coordination, activities are aimed to meet the social and emotional needs of the patient. The Care Coordinator operates in conjunction with the patient to utilize community resources to meet their individualized needs. The Care Coordinator is part of the healthcare team and indentifies and refers patients who have challenges meeting their basic needs, or who face other social issues that could negatively impact their health status. The Care Coordinator supports the patient with activates for maintenance of health and well-being.


The patients under care of this position may range in ages:

Infants & Toddler: 0 – 4

Adolescents: 15 – 18

Geriatrics: Over 65 years old

Pre-school/School age:

Adult 18 – 65 year old

JOB RESPONSIBILITIES

  • Develops and reviews service plans in collaboration with patients/families.
  • Performs follow-up re-assessment of services arranged and implemented for the quality of the services.
  • Collaborates with providers to develop an ongoing plan of services needed by patients with chronic conditions.
  • Assists patients with navigation of access processes within MHHC.
  • Works with patient/families to determine eligibility for financial assistance.
  • Acts as liaison to community organizations and agencies to establish a comprehensive network of support services to meet many social and emotional needs of the patient.
  • Collects data and participates in performance improvement processes.
  • Advocates for patients in connection with obtaining pharmaceutical and durable medical supplies.
  • Ensures patients receive transportation services permitting them to arrive to scheduled appointments
  • Evaluates patient’s independent living skills, and obtains services to support and maintain the patient in their home

JOB SPECIFICATIONS


1. EDUCATION: Graduate of a social work program, public health, public administration with a bachelor’s degree from an accredited college/university, required.
2 EXPERIENCE 1-2 years social work, public administration, public health
3 INTERPERSONAL SKILL Professional appearance and communication style.

4 REASONING ABILITY Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems.
The ability to deal with nonverbal symbolism (formulas, scientific equation, graphs, etc.

5 LANGUAGE SKILLS Ability to read, analyze, and interpret the most documents. Ability to respond effectively to the most sensitive inquiries or complaints.
Bilingual Spanish preferred. Computer literacy specifically Microsoft Word, Excel, and the electronic medical record

6 MATHEMATICAL SKILLS Ability to apply advanced mathematical concepts such as exponents, logarithms, quadratic equations and permutations

7 PHYSICAL DEMANDS While performing the duties of this job, the employee is regularly required to speak or hear. The employee frequently is required to stand; sit; use hands and fingers; handle or feel and reach with hands or arms. Specific vision abilities required by this job including close vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
