The Renal Care Coordinator (RCC) is a clinical professional embedded within a local nephrology practice, organizing care for late-stage chronic kidney disease (CKD) patients by providing support, education, and care coordination to ensure an optimal start to dialysis. Responsibilities include assessing patient knowledge of late-stage CKD and treatments, educating patients to make informed decisions, providing support for conservative or palliative care, acting as a liaison to ensure patients and families receive comprehensive information on treatment modalities and access types, organizing CKD patient education and care processes, and participating in clinical data interpretation to improve care. Requirements include a minimum of 2 years of clinical renal patient care experience, a combination of renal transplant, dialysis, or CKD patient care, understanding of diabetes and cardiovascular disease preferred, strong organizational and communication skills, RN highly preferred, and bilingual English/Spanish a plus. The position is full-time, onsite in Houston, TX, with occasional travel to satellite locations.
