You’ll work in a supportive and empowering environment where your voice and leadership matter.
- What You’ll Do
- Conduct in-home consultations with prospective clients and families
- Create and manage personalized Care Plans
- Build and maintain strong client relationships
- Conduct timely quality assurance visits (24 hours, 7 days, 30 days, quarterly)
- Serve as the go-to for client/family concerns — you’ll listen, solve, and care
- Collaborate with scheduling to ensure excellent caregiver matches
- Support caregiver performance with recruitment and training teams
- Maintain accurate records in WellSky and client files
- Participate in rotating on-call leadership support
- What You Bring
- Bachelor’s degree in healthcare, social work, or human services (preferred)
- 2+ years in client care, case management, or home care leadership
- Excellent communication, organizational, and critical thinking skills
- Tech-savvy: Microsoft Office, CRM/EMR systems like WellSky
- Valid driver’s license, reliable transportation, and insurance

Job Type: Full-time
Pay: $50,000.00 - $60,000.00 per year
