Intensive Care Coordinator Wraparound! An Intensive Care Coordinator Wraparound facilitates the Intensive Care Coordination with Wraparound (ICCW) service for individuals, often children and families, with complex needs. This role involves coordinating care, developing individualized plans, and connecting clients with necessary services. Wellness Recovery Action Plan (WRAP) is a wellness tool anyone can use to get well, stay well, and make their life more the way they want it to be.

The candidate must be energetic, caring, and exceptionally well-organized. This role requires monthly visits to each client in the community and you must act as an advocate on behalf of the families and ensure that all appropriate services are made available.

Requirements:

• Must obtain LLBSW professional license upon hire
• Personal vehicle required to drive members to appointments
