Qualifications/Educational Requirements

1. Masters Degree from a school accredited by the Council on Social Work Education.

2. Excellent organization and communication skills.

3. Minimum of one year social work experience in a healthcare setting.

4. Excellent interpersonal and analytical skill.

Home Health experience is preferred.
