Registered Nurse – Clinical Care Coordinator – Stuart Florida

Evergreen Private Care has an immediate open position for a Clinical Care Coordinator. This Registered Nurse (RN) will complete initial and ongoing assessments to identify the physical, psychosocial and environmental needs of private pay home health clients. Nurse will develop, initiate and revise the plan of care as needed to ensure quality and continuity of care. Working closely with the Director of Nursing, clinical team and schedulers, the nurse will monitor assigned cases and help to coordinate services. Position includes participation in our after-hours clinical on-call rotation which consists of occasional weekend and holiday coverage. Those with Hospice and/ or case management experience encouraged to apply!

The right candidate will enjoy working amongst a close-knit group of nurses and support staff and will participate in weekly in-office meetings as well as orient new care providers on skills assessments.

Position is full time and requires visits to the client’s homes as well as administrative time in the office.
