Job Summary:

The CIC, ILST, and PBIS Service Provider will provide support to individuals with traumatic brain injuries and their families, in accordance with the New York State Department of Health's Nursing Home Transition and Diversion (NHTD) program. The service provider will work with individuals to develop and implement person-centered plans that support the individual's goals and preferences, and promote independence and community integration.

Responsibilities:

  • Conduct initial and ongoing assessments to identify the individual's needs, preferences, strengths, and challenges.
  • Develop and implement person-centered plans that address the individual's goals and preferences, and promote independence and community integration.
  • Provide ongoing support and coaching to individuals in their homes or community settings, as specified in the person-centered plan.
  • Assist individuals with daily living tasks such as meal preparation, medication management, and personal hygiene.
  • Provide training to individuals and their families on topics such as budgeting, household management, and communication.
  • Coordinate with other service providers involved in the individual's care, such as healthcare professionals, therapists, and case managers.
  • Provide Community Integration Counseling (CIC) which includes personalized counseling to help individuals with disabilities adjust emotionally to living in their community
  • Provide Independent Living Skills Training (ILST) to individuals to promote their independence and self-sufficiency.
  • Maintain accurate and timely documentation of services provided, progress towards goals, and other relevant information.
  • Attend team meetings and other required trainings and meetings.
  • Maintain confidentiality and adhere to ethical and professional standards.

Qualifications:

  • Bachelor's degree in social work, psychology, or a related field.
  • Experience working with individuals with traumatic brain injuries and/or disabilities.
  • Knowledge of the NHTD program and regulations.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in computer skills, including electronic health record systems.
  • Valid driver's license and reliable transportation.

Job Types: Full-time, Part-time

Pay: $30.00 - $35.00 per hour

Schedule:

  • 4 hour shift
